Removing Doom
If there is a bar on top of the status, then Doom can be removed by using Esuna or The Warden's Paean. Sometimes, monsters can cast Doom that can't be removed with Esuna but by other means, such as topping off the targeted player's HP bar to full, killing all the spawned adds, or by standing on a certain object (e.g., a white puddle). The non-Esuna methods of removing Doom will vary depending on the encounter.
In other cases, Doom cannot be removed and the player will die. In these scenarios, Doom is given as a punishment for failing certain mechanics, or from receiving two stacks of Twice-come Ruin, three stacks of Thrice-come Ruin, or four stacks of Fourfold-come Ruin. These debuffs are either given as a punishment for mechanic failure, such as in Delubrum Reginae, or to enforce ways of resolving certain mechanics, in a similar manner as Elemental Resistance Down or Physical/Magical Vulnerability Up debuffs.
